Referring Doctors
PlantMed doctors accept all patient referrals within the area of adjunct plant-based medicine. PlantMed work in conjunction with the patient’s treating Medical Specialist and General Practitioner.
All medical cannabis is applied for and prescribed under our PlantMed doctor’s provider and prescriber numbers. PlantMed doctors prescribe in the areas of plant medicine and work in a shared care role with the referring doctor/s to track patient progress, symptom relief, medication interactions, reductions, and overall patient wellbeing.
Referring doctors receive correspondence once a patient is approved for medical cannabis, at 3 months of treatment and ongoing as needed. PlantMed doctors work with the referring doctor to lower medications as required, and only with communication between patient’s referring practitioner/s.
There are a number of patient groups who have been approved for medical cannabis which include:
Any patient where conventional first-line treatment has failed or where a patient’s symptoms are not controlled.
Inclusion of the items outlined below in the referral facilitates a more streamlined application process and reduced Governmental (SAS-B) approval time:
- Patient’s condition
- List of symptoms which are not relieved by current medications/ treatments
- List all current medications/ treatments
- List past treatments and medications which have not relieved symptoms/were not effective
- Any associated adverse side effects of existing medication
- Inclusion of wording such as “Thank you for seeing my patient in regards to a trial of Medicinal Cannabis to help relieve the symptoms outlined above” or “Thank you for seeing my patient in regards to potential adjuvant treatment with Medicinal Cannabis”

We also offer proforma referral form pads to make the referral process as easy as possible, so please phone us if you would like PlantMed referral form pads posted to you.
