Not from Brisbane or QLD?
That’s ok, PlantMed works with patients Australia wide via telehealth consultations. Your initial telehealth video appointment is conducted using a simple video link, no apps are required! Follow up appointments can be either video or phone call consultations.
Becoming a Patient at
Your First 4 Months
Gather your Referral and Book an Appointment
Visit your GP or Self-Refer to PlantMed. (Patients under 18 years old require a referral. See Paediatric Shared Care Flowchart for requirements of referral). Call to book your initial appointment with one of our PlantMed doctors. ($55 non-refundable deposit at the time of booking).

First Appointment (Initial Consult)
A thorough consultation where our doctor will gather your full health history and discuss your best treatment options suitable to your specific needs, and will complete three of your applications to the TGA for approval.
1 HOUR CONSULT
Second Appointment (Titration)
Your doctor will discuss your medications and your dosage plan, and script your medications to commence your treatment.
30 MINUTE CONSULT
Patient & Care Coordinator
This appointment is booked by PlantMed in advance once you have begun treatment.
This is an important FREE appointment as part of your treatment plan with PlantMed.
Script Repeats
After 5 weeks and 11 weeks, your doctor will call you to re-script your medication, and continue monitoring the progress of your treatment.
15 MINUTES
Major Review
This consultation will take place between 16 weeks and 6 months into your treatment at your doctor’s discretion, based on the complexity of your condition. We track our patients’ clinical outcomes and send a letter to your referring doctor. If our doctor is satisfied, we transfer you to nurse care plan.
30 MINUTES
Book as Required
After your major review, you can book in for an appointment at PlantMed to top up your scripts as needed.
This will be the responsibility of the patient to book appointments when required, please allow up to 4 days for medication to be confirmed and dispensed.
Transferring to
Patients transferring from another clinic or prescribing doctor within twelve months of treatment
Book Your Transfer
Call our friendly team to book your initial appointments. Gather any relevant information about your previous treatment and provide it to us prior to your nurse consult.
Nurse Consult
A PlantMed nurse will ask qualifying questions about your health history and previous treatment, in preparation for your titration appointment with our doctor, who will review this information and submit 3 applications to the TGA.
45 MINUTES
Second Appointment (Titration)
The doctor will review notes from the nurse consult and discuss best treatment options for your specific needs, and your medications and dosage plan. They will script your medications to commence your treatment.

FREE APPOINTMENT

Please call to book an appointment with one of our doctors or book via our website. In order to secure your time with the doctor a $55 non-refundable booking deposit is required, and this amount is deducted off your initial consultation. An invoice for the balance of the consultation will be emailed to you for payment, which is payable within 7 days.
DVA:
For patients eligible for DVA funding, a formal application is required and a specialist may be needed in some cases. Please speak to our friendly team for more information tailored to your treatment.
Patients can self-refer to the clinic or you can present with a referral. Referrals are preferred in order to keep your treatment team up to date but not necessary for you to start treatment here at PlantMed. We offer in person or telehealth consults Australia-wide.
